FORMS
The deadline for submitting your (1) Report, (2) Annex-4 Evaluation Form, and (3) Annex-5 Attendance Sheet is the last day of your internship. The Annex-4 Evaluation Form and Annex-5 Attendance Sheet must be submitted in a sealed envelope, signed and stamped. Students conducting their course outside the city may send scanned, signed versions of their reports to their academic advisors via email. The scanned and signed Annex-4 and Annex-5 documents should be forwarded to the advisors' email addresses through their supervisors. Students who submit their documents digitally due to being outside the city are required to submit the original versions of all documents at the beginning of the academic term.
*When filling out the forms, pay attention to the course code (check it in the OBS system).

Annex-6 Insurance Waiver Petition
The "Report Draft" that you are required to complete and submit at the end of the Workplace Practice is provided at the link below. The report must be filled out using this form. **The course code and title must be updated appropriately for each semester.

Endüstriyel Tasarım Bölümü İş Yeri Dersi Kapsamında Alınacak Öğrenciler için koşullar:
- Kısmi zamanlı asistan öğrenci olmak ya da,
- 3.5 ve üzeri GNO sahibi olmaktır.
Kontenjan 2 kişi ile sınırlıdır.
